Accommodative Services

Documentation of Learning Disabilities

Students are responsible for submitting a psychological, psychoeducational or neuropsychological evaluation that:

  • Is completed by a licensed and/or certified professional who is qualified to diagnose learning disabilities and offer recommendations
  • Contains the evaluator's name(s) and title(s), testing date(s), and student's age and grade level
  • Contains current information (usually not more than one year old)
  • Is based on a battery of instruments which address aptitude, academic achievement, and information processing (ADD/ADHD diagnosis must be based on DSM-IV criteria)
  • Includes test scores, other relevant data, and interpretations
  • Clearly states the diagnosis of a learning disability and/or ADD or ADHD if warranted
  • Recommends accommodation if warranted
  • Explains connection between recommended accommodation and assessment results

Documentation of Disabilities other than Learning Disabilities

Students are responsible for submitting an assessment report that:

  • Is completed by a qualified professional, such as a medical doctor or licensed psychologist
  • Contains the evaluator's name and title, assessment date, and student's age and grade level
  • Contains current information (usually not more than one year old)
  • Identifies the disability and describes the limits it imposes
